Fire Service. In cases of fire, all employees must assist in preventing destruction of the Company's property. Fire apparatus must not be removed from the place or used, except in case of fire. The Company will train employees in this area as it deems necessary.

Fire Service. The said Owner(s), its (their) heirs, successors, administrators and assigns forever, agree to jointly maintain said service line(s) according to the conditions listed below. For purposes of this agreement, the term “maintenance” shall include maintenance, repair and/or replacement as necessary for the water service(s) to continue functioning as originally intended. This Agreement shall be binding upon the heirs, successors, administrators, and assigns of the Owner(s), over, under and upon certain real estate owned by the Owner(s), and being described as follows: AND WHEREAS the Xxxxxx County, Ohio, Board of County Commissioners, hereinafter called the County, will allow this arrangement on a case by case, ongoing basis, provided that the following requirements are continuously observed: 1. The Owner(s) of the buildings and/or parcel(s) described above shall share equally in the cost of any and all maintenance of all water service line(s) that jointly serve said buildings and/or parcels providing the service(s) indicated above, or they shall share in such cost as described below (indicate “N/A” in space below if cost will be shared equally: 2. Domestic Water Service Line – The Owner(s) shall provide for the maintenance of the domestic water service line(s), meter pit and associated appurtenances, backflow prevention devices, and other facilities associated with the domestic water service that jointly serve said buildings and/or parcel(s). It is understood by the Owner(s) that the County assumes no responsibility for the maintenance, repair or replacement of these private facilities or for any other related costs.

Fire Service. The FIRE DISTRICT shall pay the CITY annually an amount equal to the number of persons living in the unincorporated areas of Zone 8 multiplied by the CITY’S fire department budget per capita rate. i. The CITY’s fire department per capita rate is calculated based on the CITY Fire Department’s prior year budget exclusive of capital budget, and revenue from grants, wildland deployments, this and other contracts for fire service. ii. The population of the unincorporated areas served by the CITY is calculated by multiplying the number of unincorporated residential units in the CITY’s Zone (8) by 3.2. The COUNTY will provide the CITY the number of residential units in Zone 8.

Fire Service. A. The APPLICANT shall install a BWL approved backflow prevention device after the point of entry of the fire service. The list of BWL approved devices is shown on construction standard W-07-0010, and is available upon request. The bypass of the backflow prevention device shall include a 5/8” meter which will be supplied and installed by the BWL. The APPLICANT is responsible for connecting the backflow prevention device to the service piping at this space. The APPLICANT is responsible for the ownership and maintenance of the back flow prevention device, and all piping downstream thereof. B. If a fire pump is proposed, a certified pump characteristic curve must be submitted to the Designer Representative for review and written approval. C. A monthly charge in accordance with the current BWL rates for fire services entering the building and for on- site fire hydrants (private fire hydrants) shall be applicable. D. The Fire Service and installed backflow prevention device must be inspected by the BWL prior to any turn- on. Contact the Water Distribution Department at 702-6490 for inspection. E. An adequate space must be provided in the building, or in a BWL approved manhole, for the back flow prevention device installation. The space shall be readily accessible, well ventilated, and protected from the weather elements. F. Piping for fire services and back flow prevention devices shall be installed with a minimum clearance of 18" from the nearest wall or floor, in order to prevent damage to BWL facilities. G. The APPLICANT is responsible for providing a sufficient space that is suitable enough for all fire services prior to the time of construction. Construction shall not commence, and service shall not be turned on until the APPLICANT provides a proper space that contains a fire service. If multiple fire services are proposed or already exist at the property, the APPLICANT shall be held responsible for indicating the service addressing scheme on a drawing. The APPLICANT will also be responsible for clearly labeling the piping in the field with correct addresses, prior to activation of service. ATTACHMENT C
